Templates are complete forms: questions, logic, scoring, photos and a theme. Using one makes a copy in your account; nothing you change affects the template.

  1. Select Templates in the sidebar.
  2. Search, or narrow the list with Goal, Type, Role and Length (Short · up to 5, Standard · 6–10, Long · 11+). Sort by Latest, A–Z or Quickest first.
  3. Open a template to preview it. The preview runs the real form, so you can answer it.
  4. Select Use this template. The copy opens in the builder.

What changes when you use one

  • The form gets your name for it and a fresh link when you publish.
  • A booking question in the template uses your own time zone, taken from your browser.
  • Using a template counts as one of your active forms (3 on Free).

Templates on the website

Every template also has its own page at formsai.dev/templates, with the live form to try. Signed-in visitors can open it straight in the app.
