The four screens that aren't questions: a first impression, a note between questions, a titled section and the thank-you.

ScreenUse it for
Welcome screenThe first thing people see: a title, a line on how long it takes, and a Start button. A form has one at most.
StatementText with no answer, such as instructions or a section break, with a Continue button.
Question groupA titled section. Questions you put in it are numbered 3a, 3b… See question groups.
EndingThe thank-you screen. A form can have several.

Several endings

Add more than one ending to say different things to different people, then use logic to jump to the right one (for example a quiz result, or “you qualify” and “sorry, not this time”). Without logic, people see the first ending.

Ending buttons and redirects

Select an ending to see its settings:

  • Button label adds a button, for example “Visit our site”. It's shown only together with a link in Redirect to.
  • Redirect to sends people to a web address. With a button label they click the button; without one they're redirected after 2 seconds.

There's also a form-wide redirect after the last screen in Share → Settings.

Media on any screen

Every screen can carry an image, a video or an embed, in five layouts. See layout and media.
